Enrollment Instructions
1. Begin with Colours Online Enrollment Form – You will see an explanation of benefits packages below the form to reference when selecting your Medical/Dental Coverage.
2. Make sure to SUBMIT the Form – This online form will be sent directly to Amy Wolsieffer for review. All other forms need to be downloaded, electronically filled out, and emailed to Amy!
3. Fill out the necessary forms below based upon your selections. Medical Insurance = HighMark BlueCross BlueShield. Dental Insurance = Guardian.
Colours 2026-2027 Enrollment Form
Open enrollment is the only time when you can make enrollment changes, drop coverage, or add/delete dependent(s) from your coverage, unless a qualified change in a family status occurs. All elections remain in effect for the duration of the plan year. IMPORTANT: If you're NOT making any changes to your benefits for the new plan year, then you do NOT need to fill out this form. If you are making ANY changes or are new to the plan, please fill out below.
Electronic Forms for Insurance Enrollment
The Highmark BlueCross BlueShield Enrollment Form & Guardian Life Insurance Enrollment Form MUST be completed. Please follow instructions within these forms to see if you need to fill out the subsequent Spouse Medical Affidavit or EOI form.
